Here is where you will be able to download the forms for the 2018 version of the Grafton High School Soccer Alumni game taking place on August 11th, 2018.

The cost to play is $25 which will go towards a scholarship in honor of the late Craig Carlson. The cost includes a t-shirt for the game.

You can make your payment in one of several ways: PayPal, Venmo, or check.

Please have your money and Registration form returned by Friday, August 3rd. You can turn the form in via email or print off the form and mail it.

The waiver can be signed at the field on the day of the game on August 12th.
